County of Glenn

The Glenn County, California, system-of-care community will enhance and expand our current children's system of care to deliver comprehensive community mental health services for young children (ages 0-4), children and adolescents (ages 5-18), and transition age youth (ages 14-22) with serious emotional disturbances and their families. Youth with a mental health and substance abuse dual diagnosis also will be served. This project will enhance and continue to develop a community service delivery system for these children and youth, improving access to a broad array of local partner-agency services. Program goals include keeping children and youth at home with their families, in school, out of trouble with the legal system, off illegal substances, and healthy. We are committed to measuring the effectiveness of system development, improved outcomes for children and communities, and fiscal effectiveness. Youth and families will be involved in all aspects of the system of care including planning, service delivery, evaluation, quality improvement, social marketing, advocacy, and cultural competency awareness.

Humboldt and Del Norte Counties—Wraparound System of Care 

United Indian Health Services, Inc. serves as the coordinating agency for tribal and county organizations to assure an appropriate, individualized, system of care for American Indian children with serious emotional disturbances. Components of the wraparound model include: 1) developing an early-intervention and family strength-based treatment system; 2) providing continuing cultural competency training for all participants; 3) developing a transportation network to facilitate access to services or treatment; 4) evaluation; 5) early intervention through early identification via Tribal Head Start programs, public schools, probation departments, and the American Indian community; and 6) prevention through expansion of existing cultural community-based programs (Healthy Nations). The area encompassed by this project includes 4,500 square miles and an estimated 15,000 American Indians of whom 10,000 are registered clients within the United Indian Health Services, Inc.

La Familia Sana/The Healthy Family 

The Monterey County Health Department Behavioral Health Division is enhancing and expanding the existing children’s mental health system of care. We are enhancing case management services to create bridges where there are gaps in our system. We are identifying best practices and implementing them for children 0-5, adolescents in trouble with the law, and transition age youth. We are targeting these expansions for Latino youth and their families. We are significantly increasing the involvement of parents in the entire system of care. We are structuring all these under a governance system that brings key stakeholders to the table, so that the system will be sustained beyond the period of the grant.

Sacramento County OASIS Project 

The SAMHSA OASIS project stands for obtaining and sustaining independent success.  The project will serve transition age youth and their families, particularly those from the following underserved communities: Spanish-speaking; Hmong-speaking; Russian-speaking; Vietnamese-speaking; Cantonese-speaking; homeless; and LGBT (lesbian/gay/bi-sexual/transgender).  Youth and families will have a “Family Team Assistant” or FTA whose primary responsibility will be to assist the youth and family in building a family team and in developing a plan that is strength-based and family driven.  FTAs will be located in the community in organizations and places that are not traditional mental health agencies such as community centers, schools, and non-mental health CBOs.

San Francisco System of Care 

The San Francisco System of Care (SFSOC) will integrate several critical service and infrastructure improvements to improve the functioning and well-being of SFSOC clients; increase cultural competence and youth participation in the system; reduce juvenile justice recidivism; and maintain children in homes or home-like settings whenever possible. The SFSOC will provide services to all children and youth with multi-systemic involvement across all of the major youth-serving systems in the county. It will develop innovative methodologies, funding reinvestment strategies, and an institutional infrastructure that can be adapted by other jurisdictions. Elements of the SFSOC include the incorporation of youth development principles into the system of care model; expansion of intensive care management services to children age 5 and under; piloting Dialectical Behavior Therapy and Post-Traumatic Stress Disorder Treatment models; establishment of a day treatment program for juvenile justice involved youth; a focus on cultural competence that includes the development of targeted services for San Francisco's diverse ethnic communities; expansion of debriefing services in response to community violence; implementation across the system of care of the family group conferencing model, which places the family in a key care planning role; identification, implementation, and evaluation of empirically-based treatment methodologies for children with conduct disorders, mood disorders, and ADHD; implementation of a comprehensive social marketing plan; development of outreach, engagement, and service strategies for youth; an evaluation that will involve consumers in its design, provide real-time feedback for program improvement, The collection and reporting of required outcome measures, as well as those unique to SFSOC, and documenting the process of implementation to allow for future dissemination of the model; and expansion of San Francisco's Child and Family Data Archive, providing integrated access to data for care planning, policy development, and outcome evaluation.

Urban Trails 

The City of Oakland and The Native American Health Center are collaborating to implement a system of care for emotionally disturbed American Indian children and their families. The American Indian Community has developed a strategic plan that links Native American non-profit organizations, advocacy groups, and public agencies in a comprehensive, culturally competent approach to care. 

The system of care consists of several core elements: a Native Youth Commission that serves as the governing body; a Clinical Team that coordinates case management and individualized service plans for Native youth and their families; and a Training Institute that provides cultural competency training for public officials, staff members at mainstream agencies, foster parents, and Native American agency staff.
